Registration Fee |
| US$95 (per Lion, Leo and Observers)
Registration fee in Japan will be 10,000yen (Payments
in US dollars or any credit cards will not be accepted) |

Registration by Air Mail or Facsimile before the

| You can register either by airmail
or via facsimile before the deadline.
The registration form can be downloaded
from our web site. Please notice that
the full amount of the registration payment
should be made through a bank transfer
to the account shown below. Once the transfer
has been done, please submit the receipt
of remittance of the transfer with your
registration form . If you fax, please
do not mail original. If you haven't received
a confirmation from our staff within 3
weeks after submitting the registration
form, please contact the 44 th OSEAL Registration
DESK.

Registration by internet before the deadline |
You can use our On-Line
Registration program to register for yourself
or your Group. Please confirm your total
payment. After you run the On-Line Registration
program and finish to remit your total
payment by bank transfer, the 44 th OSEAL
Registration Desk will mail your official
Confirmation of Registration in about
three weeks. Please contact the 44 th
OSEAL F.O.C. if you do not receive it.

Registration on Forum |
| If you do not have time to register before
the deadline, Sep 05, you also can register
on the Forum on Oct 07-10, 2005 . To fill the
registration form on the Forum check at the
counter then enjoy the 44 th OSEAL Forum.
On site registration fee
shall be paid in Japanese yen. We will accept
cash only. We can't accept US$, credit cards
and or traveler's check. Currency exchange is
not available at the Registration desk. You
are requested to exchange your currency into
Japanese yen before leaving your country or
at the airport from where you leave.

confirmation to which the receipt of payment
was confirmed by bank transfer after the
application deadline on September 5, 2005. |

Questionnaire on transportation means |
| When you fill in the attached registration application
form, please also answer the questionnaire on the transportation
means you will use to reach the venue for the opening ceremony. |

5.
Cancellation of Registration |
If the registration is cancelled by the 5 th (Monday)
of September, you are requested to pay 2,000yen as cancellation
fee. The registration fee after deduction of 2,000yen will
be refunded to participants after the Forum. If cancelled
after the 6 th (Tuesday) of September, the registration fee
shall not be refunded. |

Fee : US$95 (per Lion, Leo and Observers) |
Attire : Tuxedo
or dark suit/ Similar formal clothes for women.
The banquet is limited to 400 persons.
The 44 th OSEAL F.O.C would like to report the information
of application method to you later. |

2.
Golf Tournament |
| Date : October
7th 2005
Time : am9:00-pm2:00
Venue : Matsushima Chisan Country Club
Fee : US$143 (per Lion, Leo and Observers)
The registration fee includes Green Fee, Caddie Fee,
and Luncheon. (Drinks are extras)
The tournament is limited to 160 persons. (first come,
first serve basis).
Application :Please apply at the same
time of Forum application. Please remit the fee together
with the registration fee. You are kindly requested
to facsimile your application form to the Forum office.

Deadline : August 5, 2005
Remarks :The free shuttle buses are
available from Headquarters Hotel to Golf Clubs and
back. |
